Job summary

South Hills Rehabilitation Center in Eugene, OR, part of Volare Health, LLC, seeks dedicated LPNs to join a supportive nursing team. Both full-time and part-time positions are available, with competitive pay and a focus on patient comfort and quality of life.

Newly licensed LPNs welcome. You’ll assess residents, implement care plans, document care, and collaborate with an interdisciplinary team in a skilled nursing setting.

Qualifications

• Current, active LPN license in Oregon.
• Ability to assess resident needs and respond to changes.
• Knowledge of individualized care plans and nursing practices.

What You'll Do

Responsibilities

• Assessing resident needs and monitoring changes in condition


• Developing and implementing individualized care plans


• Providing direct nursing care and ensuring residents' needs are met


• Supervising CNAs and other care team members in the delivery of nursing services


• Maintaining accurate and timely clinical documentation


• Following applicable state and federal regulations, facility policies, and nursing standards


• Communicating effectively with residents, families, providers, and the interdisciplinary care team


• Advocating for residents and helping promote their comfort, dignity, and quality of life

What We\'re Looking For

• Current, active Licensed Practical Nurse (LPN) license in the state of Oregon


• Ability to assess resident needs and respond appropriately to changes in condition


• Knowledge of individualized care planning and nursing practices


• Ability to supervise and work collaboratively with CNAs and other healthcare team members


• Strong communication and organizational skills


• Compassion, dependability, and a genuine desire to make a difference in the lives of others


• Commitment to following state and federal regulations governing skilled nursing care
