Royal Meadowview Nursing and Rehab – AuthentiCare Health Centers is seeking a compassionate Licensed Practical Nurse (LPN) to join our skilled nursing team in Massachusetts. You will administer medications, monitor residents, document care, and collaborate with the healthcare team to promote safety and well‑being.

Qualifications

• Active LPN license in the state of employment.
• Knowledge of nursing principles and medication administration.
• Strong observation and clinical judgment.
• Ability to report changes promptly and accurately.
• Accurate documentation and organizational skills.
• Strong verbal and written communication; multidisciplinary teamwork.
• Commitment to resident safety and confidentiality.

Responsibilities

• Provide direct nursing care per individualized care plans and policies.
• Administer medications and treatments within LPN scope.
• Monitor vital signs, symptoms, and health; report changes.
• Document care and medications in the electronic medical record.
• Update care plans with the nursing team.
• Communicate with physicians, RNs, CNAs, residents, and families.
• Assist with activities of daily living as needed.
• Follow infection prevention and safety procedures.
• Maintain HIPAA confidentiality.
• Participate in care planning and coordination to ensure continuity.
• Support hospice or end-of-life care when appropriate.
• Assist across skilled nursing, long-term, subacute, and memory care.
• Respond to emergencies per facility procedures.
• Promote dignity, independence, comfort, and quality of life.
