Job summary

Resources for Human Development (RHD) in Cedar Rapids is seeking a Licensed Practical Nurse (LPN) to join the Iowa ACT program. The role involves overseeing and coordinating health-related needs for participants as part of a multidisciplinary team providing behavioral health services.

The LPN will manage medication administration, health education, and documentation, while ensuring confidentiality and compliant care.

Qualifications

Current Licensed Practical Nurse (LPN) license.

Responsibilities

Maintain accurate nursing documentation and medication records.
Assist participants in learning about their medications.
Ensure medications are taken as ordered by physicians and properly recorded in the eMAR.
Provide nursing care and health education, including diabetes counseling.
Assess participants' health and emotional needs.
Communicate and collaborate with nursing and program staff to coordinate participant care.
Communicate serious health or program concerns to appropriate leadership.
Maintain participant confidentiality and protect private health information.
Provide direct nursing support based on individual needs.
Perform additional duties as assigned.
