Job summary

Open Sky Community Services is seeking a Licensed Practical Nurse in Massachusetts to work in a person-centered, community-based setting. You will collaborate with the RN to meet medical needs and liaise with families and other medical providers.

Responsibilities include medication management, monitoring, advocacy, and supporting health standards. ISP participation and thorough documentation are required.

Qualifications

Licensed Practical Nurse in Massachusetts required.
Excellent assessment, documentation, and verbal communication skills required.
Valid driver’s license and acceptable driving history required.
Experience with adults with disabilities preferred.

Responsibilities

Oversee medication ordering, counts and refills per physician orders.
Monitor medical status and transport to medical appointments or arrange transport.
Advocate for individuals with physicians and service providers.
Ensure health and wellness standards through collaboration with staff, including nutritious meals.
Attend ISP meetings and complete necessary ISP documentation.
