Open Sky Community Services is seeking a Licensed Practical Nurse in Massachusetts to provide person-centered care in a non-clinical setting. The LPN collaborates with the RN to support medical needs, advocates for individuals, and coordinates care with families and other providers.

The role emphasizes long-term, relationship-based care, with a focus on health, wellness, and nutritious meals, plus involvement in ISP meetings and required documentation.

Formación

• High School Diploma, GED or equivalent, required.
• Certified Licensed Practical Nurse in Massachusetts, required.
• Excellent assessment, documentation, and verbal communication skills, required.
• Valid driver’s license and acceptable driving history, required.
• Experience with adults with disabilities, preferred.

Responsabilidades

• Oversee medication ordering, counts and refills following physicians' written orders.
• Monitor medical status and transport, or arrange transport to appointments.
• Advocate for individuals with physicians and other service providers.
• Ensure health and wellness standards in collaboration with staff, including nutritious meals.
• Attend ISP meetings and complete necessary documentation for the ISPs.

Job Description:Are you a Licensed Practical Nurse looking for an environment that is not an office or hospital setting? Workdays can be a challenge at times, however, with a pace slower than a hospital setting and the ability to work with people on a long-term basis, Open Sky LPNs are provided a unique opportunity for those who are looking for a clinically based, supportive and nurturing role.The Licensed Practical Nurse works in close collaboration with the Registered Nurse to ensure the medical needs of people supported are met.
