Job summary

Open Sky Community Services seeks a Licensed Practical Nurse to support individuals in a non-clinical setting, emphasizing long-term connections and person-centered care. You will work closely with the Registered Nurse to meet medical needs while liaising with families and external providers.

You will oversee medication orders and participate in health-monitoring, care coordination, and ISP-related documentation within a supportive, trauma-informed, diverse workplace.

Qualifications

• LPN license in Massachusetts is required.
• Excellent assessment, documentation, and verbal communication skills are required.
• Valid driver’s license with acceptable driving history is required.
• Experience with adults with disabilities preferred.

Responsibilities

• Oversee medication ordering, counts and refills per physician orders.
• Monitor medical status and transport to medical appointments or delegate transport.
• Advocate for individuals with physicians and other service providers.
• Ensure health and wellness standards, including development of nutritious meals, with staff collaboration.
• Attend ISP meetings and complete necessary ISP documentation.
