• LPN license required in MA.
• Excellent assessment, documentation, and verbal communication skills required.
• Valid driver’s license and acceptable driving history preferred.

• Oversee medication ordering, counts and refills following physicians’ orders.
• Monitor health status and arrange transportation to appointments when needed.
• Advocate for individuals with physicians and other service providers.
• Support health and wellness standards including nutritious meals development.
