Phoenix Home Care and Hospice is seeking a Home Health LPN Clinical Manager in the Independence, MO area.

Responsibilities

1. Assist in the development, review, and revision of the care plan.
2. Provide nursing care according to the physician’s plan of treatment and individual care plan, as authorized by the client’s payer source.
3. Observe, record, and report reactions to treatment and any changes in the client’s condition to appropriate personnel and/or physician.
4. Offer ongoing instruction and supervision to clients, families, and other healthcare team members in the care plan.
5. Participate in education programs, quality assessment activities, nurse’s meetings, and other staff meetings.
6. Oversee patient and staff assignments.
7. Manage referrals appropriately.
8. Initiate patient discharge planning upon admission and complete discharge summaries accurately and promptly.
9. Perform supervisory patient visits as required.
10. Conduct PRN visits, work rotating weekend shifts, and be available on call as needed.
11. Engage in quality improvement activities, including chart audits and reviews.
12. Communicate patient status and issues daily with the DON.
13. Travel to patients' homes and outreach locations to provide care under the DON’s supervision.
14. Maintain compliance with all policies, procedures, and regulations, including federal and state laws.
15. Perform other duties as assigned by the supervisor.

Minimum Requirements

• Missouri or compact LPN License
• At least one year of home health experience preferred
• Minimum of one year successful experience as an LPN
• Valid driver’s license and proof of current liability insurance
• Current CPR certification
• Ability to lift 50 pounds
• Effective communication skills, both face-to-face and via phone
• Strong organizational and self-management skills
