Chronic Care Manager

Location: Remote

Join our mission to transform healthcare delivery from reactive, episodic care to proactively managed patient care, preventing life-changing problems for patients with two or more chronic conditions. We believe every patient with a chronic disease deserves consistent check-ins, follow-up, and support.

The Nurse Chronic Care Coordinator, Remote, will perform telephonic encounters with patients on behalf of our partners each month and develop detailed care plans within our electronic health record templates. This is initially an Independent 1099 Contractor position with potential to transition to full-time W2 employment with benefits.

Harris CCM seeks Nurses to work part-time from home while complying with HIPAA privacy laws. You will set your own hours, with a minimum of 20 hours per week. We value work-life balance, allowing flexibility in choosing your work days and hours. Care Coordinators will typically manage a minimum of 100 patients per month, completing encounters on 90% of their assigned patients.

We utilize a productivity-based pay structure, paying from $10.00 per encounter up to $16.00, depending on the number of encounters per month, with increases requiring three months of consistency. Each encounter takes at least 20 minutes.

What Your Impact Will Be
  • Follow the care plan and practice orders.
  • Provide prevention and intervention for multiple conditions through motivational coaching.
  • Build positive interactions with patients.
  • Enhance revenue by creating billable CCM episodes and increasing management visits.
  • Develop detailed care plans for doctors and patients, focusing on prevention and intervention.
  • Understand healthcare goals related to chronic disease management.
  • Attend scheduled meetings, such as bi-monthly staff meetings and monthly one-on-ones.
What We Are Looking For
  • Graduate of an accredited School of Nursing (LPN, LVN, RN, BSN, etc.).
  • Current compact license with no disciplinary actions.
  • At least two years of clinical experience in Med/Surg, Case Management, or home health.
  • Experience with Electronic Medical Records and Windows applications, in a HIPAA-compliant home setting.
  • Strong initiative, judgment, organization, and problem-solving skills.
  • Proficiency with computer programs; love of technology is a plus.
  • High-speed internet and a Windows or Mac desktop/laptop (no Chromebooks).
  • Excellent verbal, written, and listening skills.
