The Nurse Chronic Care Coordinator, Remote, will perform telephonic encounters with patients on behalf of our partners each month and develop detailed care plans within our electronic health record templates. This is initially an Independent 1099 Contractor position with potential to transition to full-time W2 employment with benefits.
Harris CCM seeks Nurses to work part-time from home while complying with HIPAA privacy laws. You will set your own hours, with a minimum of 20 hours per week. We value work-life balance, allowing flexibility in choosing your work days and hours. Care Coordinators will typically manage a minimum of 100 patients per month, completing encounters on 90% of their assigned patients.
We utilize a productivity-based pay structure, paying from $10.00 per encounter up to $16.00, depending on the number of encounters per month, with increases requiring three months of consistency. Each encounter takes at least 20 minutes.
