Responsibilities

St. Mary's Medical Center Physician Care is now hiring a full time day shift Licensed Practical Nurse (LPN) Care Manager to join our team! The Licensed Practical Nurse (LPN) - Care Manager duties include, but are not limited to:

• act as a liaison between patients, health care providers, and health insurers to ensure patients receive excellent health care
• provide direct and indirect patient care services requiring technical and manual skills, within the scope of practice, that meet the psychosocial, physical and general aspects of care
• maintain patient medical records and keep track of health outcomes
• communicate the needs of the patient and family
• consider the patient's unique care needs and advocate for personalized treatment options
• develop and manage long-term care plans for patients with chronic or serious conditions
• educate patients and their families on relevant health-related matters, and offering guidance for navigating complex medical decisions
• performs under the direction and supervision of a Registered Nurse
• have exceptional communication skills, organizational skills, and analytical and problem-solving skills

Qualifications

Education and Work Experience

• current and valid state license as a Licensed Practical Nurse (LPN/LVN)
• current BLS (AHA) certificate upon hire and maintain current
• minimum one year experience in an applicable setting, preferred
• IV certification upon hire
