Job Title: Home Health LPN Clinical Manager

Phoenix Home Care and Hospice is seeking a Home Health LPN Clinical Manager in the Independence, MO area.

Responsibilities

• Assist in developing, reviewing, and revising care plans.
• Provide nursing care according to physician’s treatment plan and client’s care plan, authorized by the payer source.
• Observe, record, and report treatment reactions and changes in client condition.
• Educate and supervise clients, families, and healthcare team members in treatment plans.
• Participate in educational, quality assessment, and staff meetings.
• Oversee patient and staff assignments and referrals.
• Coordinate patient discharge planning and complete discharge summaries timely.
• Perform supervisory visits, PRN visits, and be available for weekend shifts and on-call duties.
• Engage in quality improvement activities, including chart audits and reviews.
• Communicate patient status with the Director of Nursing (DON) daily.
• Travel to patients' homes and outreach locations to provide care under DON supervision.
• Ensure compliance with all policies, procedures, and regulations.
• Perform additional duties as assigned.

Minimum Requirements

• Missouri or compact LPN License
• At least one year of home health experience preferred
• Minimum of one year successful experience as an LPN
• Valid driver’s license and proof of insurance
• Current CPR certification
• Ability to lift 50 pounds
• Effective communication skills, both face-to-face and via phone
• Strong organizational and self-management skills
