The ideal candidate holds an Ohio LPN license, has strong communication and documentation skills, and can navigate Medicare/Medicaid processes, with weekend rotation in the schedule. The organization emphasizes compassionate care and teamwork.

Qualifications

• Must possess current LPN license in Ohio.
• Experience in a medical facility or senior living setting preferred.
• Fluency in reading, writing, and speaking English.
• Ability to pass a pre-employment background check and drug screen.

Responsibilities

• Attends clinical meetings for continuity of care.
• Participates in leadership meetings.
• Achieves or exceeds budgeted occupancy in skilled nursing.
• Supports CMS Quality Measure outcomes.
• Completes resident interviews and assessments.
• Provides consultation to staff and coordinates with community agencies.
