UnitedHealth Group is seeking a Clinical Care Coordinator LPN to support post-acute care pathways across hospitals, physicians, and families. The role focuses on coordinating patient visits, referrals, and care plans to maintain continuity and quality of care in the home setting.

The position emphasizes collaboration with RN Case Managers and Social Workers, ensuring regulatory compliance while supporting a compassionate care approach for patients at various stages of need.

Formación

• Current LPN/LVN license in state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire (preferred).

Responsabilidades

• Helps to manage order tracking, IDG preparation, and other tasks associated with the management of patient care.
• Monitors pending referrals daily and assigns RN or APRN for start of care visits.
• Monitors productivity and distribution of visits and communicates changes with the clinical leader as appropriate.
• Ensures patient visit schedule supports continuity of care by scheduling nursing visits to the designated RN Case Manager and Social Worker.
• Participates in patient care team meetings to facilitate timely care for patients in crisis or end-of-life transitions.
• Assists with coordination, scheduling, and tracking of all visits to ensure compliance with hospice policies and regulations.
