Responsibilities

• Assist Patient Care Manager in coordinating staff to ensure high-quality care per physician orders and regulations.
• Prepare patient lists and necessary documents for weekly case conferences.
• Provide direct patient care as needed within the plan of care and physician orders; coordinate with the health care team.
• Assist in clinician documentation review to ensure complete and accurate submissions by field staff.
• Operate in diverse home settings, respecting patient diversity.
