Qualificações

• Graduate of an accredited practical nursing program and licensed to practice in NYS.
• Minimum of 4 years of experience in a clinical setting.
• Exceptional customer service and communication skills.
• Must be highly proficient with computers, detail-oriented, and able to work in a fast-paced environment.

Responsabilidades

• Cultivate relationships with physicians and hospitals to coordinate home care solutions.
• Monitor RightFax folders, label faxes, and attach documents to patient records in HCHB.
• Monitor Intake Team email and triage requests.
• Enter orthopedic prescreens into HCHB per agency protocol.
• Provide phone support and triage calls to Intake staff.
