Signature HealthCARE in North Carolina is seeking an LPN/LVN to provide direct nursing care to residents in a 106-bed facility. You will supervise nursing assistants, administer medications, and coordinate care plans to maintain high-quality outcomes.

Qualifications

• LPN/LVN license active in the state of practice.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory/charge nurse role preferred.
• Strong observation, medication administration, and documentation skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs, ensuring comfort, dignity and safety.
• Supervise and direct nursing assistants and medication aides during shift.
• Monitor residents’ status and notify physicians of changes.
• Develop, implement, and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance and regulatory activities.
• Perform other duties as assigned by Nursing leadership.
