Job summary

Autumn Lane Health & Rehab in Georgia is seeking a Licensed Practical Nurse (LPN) to join our compassionate care team. You will provide high-quality clinical care, collaborate with RNs, CNAs and the interdisciplinary team, and deliver person-centered care in a supportive environment.

We offer $26.25 to $29.50 per hour, evening/night/weekend shift differentials, weekly pay, and benefits including a 401(k) with match, health/dental/vision coverage, PTO and holiday pay, plus employee recognition

Qualifications

• Active LPN license in Georgia or eligibility.
• Compassionate, high-quality care.
• Strong clinical judgment and teamwork.
• Reliable and committed to excellence.
• Prior long-term care experience preferred.

Responsibilities

• Administer medications and treatments as prescribed
• Monitor residents' conditions and report changes to the care team
• Provide direct nursing care in accordance with care plans
• Supervise and support CNAs in delivering quality care
• Maintain accurate and timely documentation
• Ensure compliance with all clinical policies and regulatory standards
• Communicate effectively with residents, families, and team members

Your Impact as an LPN
