Job summary

Signature HealthCARE of Terre Haute is seeking a licensed LPN/LVN to provide direct nursing care to residents in a 176-bed facility. You will administer medications and assist with ADLs, ensuring patient comfort, dignity and safety.

You will supervise nursing staff, document care in the EHR, and collaborate with the interdisciplinary team to coordinate admissions and discharges while maintaining high care standards.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with EHR systems and documentation.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies.
• Monitor and observe residents’ physical and emotional status; notify the physician of changes.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate resident admissions, transfers, and discharges.
• Participate in quality assurance activities and regulatory surveys.
