Job summary

Signature HealthCARE in Terre Haute is seeking an LPN/LVN to provide direct nursing care, assist with ADLs, and supervise nursing assistants on assigned shifts. The role emphasizes accurate documentation, patient dignity, and safety within a patient-centered environment.

You will develop and review care plans with the interdisciplinary team, coordinate admissions/transfers, and participate in quality assurance activities. Strong communication and regulatory knowledge are essential.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents' status; notify physician and responsible party of changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities, staff education, and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
