Job summary

Signature HealthCARE of Terre Haute is seeking a dedicated LPN/LVN to provide direct nursing care to residents, administer medications, perform wound care, and support skilled nursing procedures in a 176-bed facility.

You will supervise nursing assistants and medication aides, coordinate admissions and discharges, maintain accurate documentation, and participate in QA activities to ensure high-quality, person-centered care.

Qualifications

• Must hold current LPN/LVN license in the practicing state.
• Minimum 1 year of clinical nursing experience; LTC experience preferred.
• Experience in supervisory or charge nurse roles preferred.
• Strong observation, medication administration, and documentation skills.
• Familiarity with EHR systems and MS Office.

Responsibilities

• Provide direct nursing care, including medication administration and wound care.
• Assist residents with ADLs ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides per policies.
• Monitor residents' status and notify physician of changes.
• Develop and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, discharges per procedures.
• Participate in QA activities, staff education, and regulatory surveys.
• Perform other duties as assigned by DON.
