Signature HealthCARE of Terre Haute is seeking an attentive LPN to provide direct nursing care in our 176-bed facility. You will administer medications, assist with ADLs, and help ensure residents' comfort, dignity, and safety.

You will supervise nursing assistants on shift, maintain accurate documentation, and collaborate with an interdisciplinary team to develop individualized care plans. Indiana license and CPR/BLS are required or preferred; long‑term care experience is a plus.

Qualifications

• Active LPN/LVN license in Indiana.
• Minimum 1 year of clinical nursing experience; long-term care preferred.
• Supervisory or charge nurse experience preferred.
• Familiarity with EHR systems.
• Proficiency in Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents' status and notify physician of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
