Job summary

Signature HealthCARE Terre Haute is seeking a caring LPN/LVN to provide direct nursing care to residents, administer medications, manage wound care, and support daily living activities in a 176-bed facility.

The role includes supervising nursing assistants, maintaining accurate documentation, coordinating admissions and transfers, and participating in quality assurance and regulatory activities while upholding patient dignity and safety.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.
• Strong verbal and written communication skills.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the assigned shift.
• Monitor and observe residents; report changes to physician and responsible party.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in quality assurance activities and regulatory surveys.
