Job summary
Signature HealthCARE Terre Haute seeks a dedicated LPN/LVN to provide direct nursing care, including medication administration and wound care, while supporting residents with daily activities and safety. You will supervise nursing assistants, document care, and collaborate with a multidisciplinary team to optimize resident outcomes.
The role requires an active LPN/LVN license, at least 1 year of nursing experience (long-term care preferred), and familiarity with EHRs and state/federal
Qualifications
- Active LPN/LVN license in the state of practice.
- At least 1 year of clinical nursing experience, long-term care preferred.
- Supervisory or charge nurse experience preferred.
Responsibilities
- Provide direct nursing care including medication administration and wound care.
- Assist residents with activities of daily living (ADLs) ensuring comfort and safety.
- Supervise nursing assistants and medication aides during shifts.
- Monitor residents and notify physicians of changes in condition.
- Develop and review individualized care plans with the interdisciplinary team.
- Complete accurate clinical documentation per policy and procedure.
- Coordinate admissions, transfers, and discharges per procedures.
- Participate in quality assurance activities and regulatory surveys.
