Stevens Park Health & Rehab in Augusta, GA is seeking an LPN to join a mission-driven nonprofit that provides high-quality, person-centered care. You’ll work with RNs, CNAs, and the interdisciplinary team to deliver compassionate nursing services in a resident-centered environment.

Qualifications

• Active Licensed Practical Nurse (LPN) license (State of Georgia or eligible).
• Strong clinical judgment and critical thinking skills.
• Excellent communication and teamwork abilities.
• Reliability and a commitment to excellence.
• Prior long-term care experience preferred, but not required.

Responsibilities

• Administer medications and treatments as prescribed
• Monitor residents’ conditions and report changes to the care team
• Provide direct nursing care in accordance with care plans
• Supervise and support CNAs in delivering quality care
• Maintain accurate and timely documentation
• Ensure compliance with all clinical policies and regulatory standards
• Communicate effectively with residents, families, and team members
