Job summary

Riverside Health & Rehab in Connecticut is seeking a Licensed Practical Nurse (LPN) to join a compassionate, patient-centered care team. You will work closely with RNs, CNAs, and the interdisciplinary team to provide high-quality clinical care and ensure residents’ well-being.

The role emphasizes administering treatments, monitoring conditions, and ensuring accurate documentation while upholding regulatory standards in a supportive environment.

Qualifications

• Active Licensed Practical Nurse (LPN) license (State of Georgia or eligible).
• A passion for delivering compassionate, high-quality care.
• Strong clinical judgment and critical thinking skills.
• Excellent communication and teamwork abilities.
• Reliability and a commitment to excellence.
• Prior long-term care experience preferred, but not required.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents’ conditions and report changes to the care team.
• Provide direct nursing care in accordance with care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with all clinical policies and regulatory standards.
