Job summary

Lynn Haven Health & Rehab in Georgia seeks an experienced Licensed Practical Nurse (LPN) to deliver high-quality clinical care as part of a dedicated interdisciplinary team. You will work closely with RNs, CNAs and other staff to implement care plans and ensure residents receive compassionate, person-centered care.

Qualifications

• Active Licensed Practical Nurse (LPN) license (State of Georgia or eligible)
• A passion for delivering compassionate, high-quality care
• Strong clinical judgment and critical thinking skills
• Excellent communication and teamwork abilities
• Reliability and a commitment to excellence
• Prior long-term care experience preferred, but not required

Responsibilities

• Administer medications and treatments as prescribed
• Monitor residents’ conditions and report changes to the care team
• Provide direct nursing care in accordance with care plans
• Supervise and support CNAs in delivering quality care
• Maintain accurate and timely documentation
• Ensure compliance with all clinical policies and regulatory standards
• Communicate effectively with residents, families, and team members
