Job summary

Southeastern Virginia Training Center (SEVTC) seeks a dedicated Licensed Practical Nurse (LPN) Case Manager to provide professional nursing services and clinical case management for individuals with IDD in an ICF/IID setting.

As a primary nursing resource, you will advocate for residents and collaborate with families, physicians, nurses, therapists, DSPs, QIDPs, and the interdisciplinary team to support person-centered care.

Qualifications

• Knowledge of practical nursing principles and practices.
• Strong organizational, communication, documentation, and interpersonal skills.
• Ability to establish priorities and manage an assigned caseload.
• Ability to perform nursing tasks within LPN scope of practice.
• Basic computer skills and electronic health record proficiency.
• Current Virginia LPN license or Nurse Licensure Compact state.

Responsibilities

• Coordinate nursing services and manage an assigned caseload.
• Monitor health status and report significant changes to RN/physician.
• Administer medications per physician orders.
• Monitor medication effectiveness and side effects.
• Assist with medication reconciliation after hospitalizations or changes.
• Provide nursing input into care plans and ISP meetings.
• Collaborate with IDT on health-related ISP components.
• Participate in medication reviews and QA activities.
• Serve on Pharmacy and Therapeutics Committee as assigned.
• Attend required meetings, training and continuing education.
