Job summary

Sevita’s Mentor Community Services is hiring a Nurse Case Manager (IDD) in a hybrid role serving the Lowell area. The LPN or RN licensed nurse will develop and implement home-care plans ensuring services meet quality standards and licensing guidelines.

Responsibilities include intake, care planning, data review, staff training, and cross-team coordination. Required are a valid nursing license, 2+ years of related experience, a valid driver’s license with a reliable vehicle, strong communication

Qualifications

• Valid LPN or RN Nursing License.
• At least two years of relevant experience.
• Valid driver’s license with 1+ years' driving experience.
• Vehicle - reliable insured/registered vehicle.
• Self-motivated and detail-oriented with ability to multi-task.
• Excellent communication and analytical skills.

Responsibilities

• Perform intake assessments and assist with prior authorization documentation.
• Develop a care plan based on nursing diagnosis and input from individuals and physicians.
• Review medical data to assess rehabilitation potential.
• Investigate and address concerns/complaints.
• Ensure aides are oriented and trained per policy.
• Conduct supervisory visits with written evaluations for aides.
• Educate individuals and caregivers to maintain or achieve optimal functioning.
• Coordinate home care services and transfer medical data to physicians/therapists.
• Provide input for policy formulation and participate in QA boards.
