Job summary

Paycom - ATS is seeking a Case Manager (LPN) in Youngstown, OH. The CM will manage and coordinate services to support clients living safely in home-based settings under supervision, with required travel across four counties and occasional travel outside the area.

Key requirements include holding an LPN license, OSHIIP certification within one year, valid driver’s license, background check, and drug screen. Proficiency with data entry and Excel is expected.

Qualifications

• Must hold a Licensed Practical Nurse (LPN) license with oversight from licensed staff.
• Ability to provide assessments compliant with client needs and regulations.
• OSHIIP certification must be obtained within one year of hire.
• Must have a valid driver’s license and proof of auto insurance.
• Must pass a criminal background check and drug screen.

Responsibilities

• Coordinate services to support clients living in a Home and Community Based Setting.
• Effectively communicate with clients in person, by phone, and via email.
• Document facts accurately and enter data in spreadsheets.
• Travel to community and consumer locations within four-county area (Ashtabula, Mahoning, Trumbull, Columbiana).
• Occasionally travel outside the four-county area, including Columbus, OH.
