The Care Team Hospice is seeking a compassionate Hospice RN Case Manager in the Youngstown, OH area to coordinate patient care and develop individualized plans of care. You will collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life for patients and families served at home.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards.
• Complete scheduled visits, admissions, and interventions with accurate documentation in HCHB.
• Respond to patient, family, and caregiver needs, including after-hours coverage.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care.
• Educate patients and families on symptom management and end-of-life care.
