Job summary

Paycom - ATS seeks an Assessor to coordinate person-centered care plans and manage LOC assessments within HCBS settings. You will work under supervision to ensure services meet client needs and live safely in home-based settings.

Responsibilities include conducting LOC assessments, re-determinations, and coordinating referrals while traveling to consumer locations across four counties, with possible travel to Columbus. Requires relevant healthcare experience and proper licensure or education.

Qualifications

• Prefer LPN or BA/BS with 1 year relevant experience.
• Experience with information and referral or long-term care options.
• Must pass a criminal background check and drug screen.
• OSHIIP certification must be obtained within one year of hire date.
• Valid driver’s license and proof of automobile insurance.

Responsibilities

• Assess and coordinate initial person-centered care plans (PCCP) and PCSP within designated cost caps.
• Conduct LOC assessments and redeterminations for MyCare Ohio Waiver program.
• Travel to community and consumer locations within four-county service area and occasionally outside it.
