Job summary

Vitra Health seeks a licensed practical nurse (LPN) to provide nonclinical support at clients’ homes across Massachusetts. You will educate clients and caregivers on health maintenance, assist in healthcare navigation, and access community resources.

Responsibilities include intake assessments, in-home visits, care-plan reviews, multidisciplinary collaboration, and thorough documentation.

Qualifications

• Must be licensed by the Massachusetts Board of Registration in Nursing.
• At least two years of direct care experience with elders or adults with disabilities.
• Excellent interpersonal and communication skills required.
• Valid driver’s license with proof of insurance.
• Bilingual in English and a secondary language preferred.
• Experience in healthcare, home health care, case management, or sales is preferred.

Responsibilities

• Complete intake and annual assessments and admission visits with nursing.
• Conduct monthly in-home visits and caregiver education.
• Perform semi-annual visits to review plans of care and goals with nursing.
• Assist clients in accessing information and community services.
• Participate in multidisciplinary care planning and discharge planning.
• Communicate with clients, PCPs, and other providers as needed.
• Complete required documentation per regulations.
