Qualifications

Responsibilities

• Complete intake and annual assessment and admission visits with nursing.
• Conduct semi-annual visits and update plan of care.
• Assist with obtaining information and accessing healthcare services.

Skills

Excellent interpersonal and communication skills

Bilingual in English and secondary language

Licensed by the Massachusetts Board of Registration in Nursing
