Job summary

True Care seeks an experienced LPN Supervisor to oversee clinical review staff and coordinate daily operations for our NY home care program. Working under the Director of Patient Services, you will ensure compliance, maintain accurate records, and support a team delivering high-quality patient care.

Location is on-site in Brooklyn, NY. The role requires strong organizational skills, home care experience, and the ability to lead clinical staff in a fast-paced environment.

Qualifications

• Current, unrestricted LPN license in New York State.
• Minimum 3 years LPN experience, incl. 2+ years in home care (LHCSA/CHHA/MLTC).
• Minimum 1 year in lead/coordinator/supervisory role.
• Working knowledge of NYSDOH home care regulations, aide compliance, Medicaid/MLTC processes.
• Proficiency with HHAExchange, EVV, and MS Office.

Responsibilities

• Supervise clinical reviewers and support staff in reviewing patient records and documentation.
• Ensure documentation aligns with RN plans of care, physician orders, and agency policy.
• Identify gaps and elevate issues to the RN supervisor.
• Manage daily workflow, staff assignments, and productivity targets.
• Coordinate with intake, scheduling, MLTC/managed care, and authorization teams.
• Monitor EVV and visit verification exceptions for timely resolution.
• Support NYSDOH regulatory compliance and audits.
