Job summary

Infinite Medical P.C. seeks an on-site Nursing Care Coordinator RN/LPN to review chart documentation and ensure continuity of care for residents in skilled nursing facilities. This role emphasizes compliance with CMS guidelines and accurate medical records.

You will work with HR, Operations, and Clinical teams, conducting audits, improving documentation, and supporting care management across multiple platforms in a fast-paced environment.

Qualifications

• CMS guidelines for chronic care documentation.
• ICD-10 diagnosis coding fundamentals.
• RVL review and clinical documentation review.
• Proficient with standard office software.

Responsibilities

• Compliance & Records Review: evaluate patient records and RVLs for accuracy and CMS adherence.
• Quality Assurance: identify discrepancies and suggest clinical corrections.
• Cross-Functional Communication: coordinate with HR, Operations, and Clinical teams.
• Professional Development: attend meetings, complete post-training surveys, cross-train.
• Systems Utilization: maintain proficiency in PCC, MatrixCare, SigmaCare and Google Workspace.
