Eatonton Health & Rehab is seeking an experienced LPN to join our team as a nursing support leader. You will facilitate communication across departments, participate in patient assessments, and help develop comprehensive care plans aligned with patient wishes and regulatory standards.

The role requires an active LPN license in Georgia and a nursing degree, with responsibilities including coordinating care, supervising care planning activities, and supporting the interdisciplinary team to ensure

Qualifications

• Nursing Degree from an accredited college or university or a graduate of an approved LPN program.
• Active LPN license to practice in the State of Georgia.

Responsabilités

• Facilitates communication between nursing center departments and patient/family to provide an effective care plan.
• Assists Director with patient/family interviews for assessment and with evaluating patient conditions to determine needs.
• Coordinates development of care plans with measurable goals and timely assessments.
• Ensures assessments and care plans are signed, dated, and compliant with regulations.
• Monitors MDS process accuracy and supports interdisciplinary team education.
