Eatonton Health & Rehab in Eatonton, GA is seeking a dedicated LPN Assessment Coordinator to join our nonprofit, mission-driven team. The role involves coordinating assessments and care plans, interfacing with patients, families, and the interdisciplinary team to ensure timely, compliant care.

Qualifications

• Nursing degree from an accredited college or university or graduate of an approved LPN program.
• Active LPN license to practice in Georgia.

Responsabilités

• Facilitates communication between nursing center departments and patient/family to support assessment and care plans.
• Establishes open lines of communication with consultants and interdisciplinary team members.
• Assists Director with patient interviews and evaluation of condition and medical data to determine needs and goals.
• Maintains assessment/care plan meetings and coordinates schedules with staff, family, and therapists.
• Ensures care plans and assessments are completed in a timely manner and compliant with regulations.
