Qualifications

• LPN education or nursing degree from an accredited program.
• Active Georgia LPN license required to practice.
• Strong communication and organizational abilities.

Responsibilities

• Facilitates communication between nursing center departments and patient/family to support assessment/care plans.
• Interviews patients, families, and staff to collect information for assessments.
• Assists Director with patient evaluations and care plan development.
• Coordinates care plan meetings with staff and families.
• Ensures care plans and assessments are timely and properly signed.
