Qualifications

• Nursing degree from an accredited college or university or completion of an approved LPN program.
• Active LPN license to practice in the State of Georgia.
• Strong communication and organizational abilities are required.

Responsibilities

• Facilitates communication between nursing center departments and patient/family members for effective assessment and care planning.
• Establishes open lines of communication with consultants and interdisciplinary team members.
• Performs patient and family interviews for assessment purposes with strong interviewing skills.
• Assists Director with evaluating patient conditions and updating care plans as needed.
• Coordinates development of preliminary and comprehensive care plans with measurable goals.
• Ensures assessments and care plans are completed within regulatory timeframes.
• Educates staff on care plans and reviews them prior to administering care.
• Maintains accurate review of assessments for timeliness, signatures, and dates.
