Job summary
InVision Customized Services is seeking a Licensed Practical Nurse (PA) to coordinate medical care and support for individuals with IDD. The role involves liaising with physicians and providers, educating staff on diagnoses, medications, and dietary guidelines, and developing standardized care protocols.
The position requires current PA LPN licensure, experience with medically and behaviorally complex cases, and proficiency with Electronic Health Records (EHR).
Qualifications
- Current PA Licensed Practical Nurse.
- Experience with IDD population is a plus.
- Experience with medically and behaviorally complex cases is a plus.
- Experience with Electronic Health Records (EHR).
Responsibilities
- Review medical history and records related to diagnoses and health.
- Coordinate care with physicians and providers.
- Serve as liaison between internal teams and providers.
- Provide training on medical diagnoses, medications, and dietary guidelines.
- Develop standardized protocols for IDD-related diagnoses.
- Observe and prompt medical interventions as needed.
- Participate in health goal development and reporting.
- Assist with vital signs and basic assessments.
- Accompany people to medical appointments.
- Support compliance with 6100 and 6400 regulations.
- Coordinate with external medical professionals and agencies.
- Advocate for people served during hospitalization.
- Serve as on-call nurse, as needed.
