VISION

Job summary

Mental Health Association, Inc. is seeking a Licensed Practical Nurse to oversee and support the on-site care team, coaching staff, and modeling best practices.

The LPN will advocate with medical providers, coordinate with visiting nursing services, and participate in discharge planning after hospital or rehab stays. The role requires a MA license, a valid driver’s license, and MAP/CPR/First Aid certifications.

Qualifications

• Must be a Licensed Practical Nurse (LPN) with an active Massachusetts license.
• Valid driver’s license and reliable vehicle.
• Strong ability to work independently while prioritizing multiple tasks efficiently.
• Proficient written, verbal, and organizational skills.
• Adaptability to meet changing needs of program participants.
• Demonstrated compassion, patience, and strong interpersonal skills.
• Must be able to obtain and maintain MAP, CPR, and First Aid certifications.
• Must be at least 18 years old.

Responsibilities

• Coordinate care with the care team on-site, providing hands-on education, coaching, and modeling.
• Monitor participants for medication effects and document findings according to protocols.
• Act as liaison between participants and external providers including hospitals, clinics, and families.
• Respond to medical or behavioral emergencies per policies; communicate with supervisors and emergency personnel.
• Report and document suspected abuse or neglect following mandated reporting procedures.
• Assist in scheduling, coordinating, and attending medical, dental, and psychiatric appointments; administer medications per MAP and record health data.
• Conduct medication log audits to ensure MAP compliance and accurate records.
• Participate in team meetings, supervisions, and ongoing training to enhance skills.
• Maintain professionalism and organizational capabilities to meet participant goals and program outcomes.
