AltaPointe is seeking a qualified Care Manager to provide clinical, care coordination and discharge support in our multidisciplinary team. The role includes chart reviews, referrals, and linkage management to ensure timely follow up and continuity of care.

You will monitor engagement, support treatment planning, and collaborate with internal programs and external providers to meet patient needs and improve outcomes.

Formación

• Strong knowledge of behavioral health systems including mental health and SUD.
• Proficiency in navigating and documenting in electronic health records.
• Experience with treatment planning and interagency coordination.
• Excellent organizational and communication skills, with attention to documentation.
• Ability to work independently and in a team.

Responsabilidades

• Chart Review and Documentation in the Care Manager System.
• Coordinate physical, behavioral, and social health services across programs.
• Facilitate access to housing, benefits, employment supports, and SUD care.
• Ensure referrals are generated, tracked, and closed with documentation.
• Follow up within 24 hours of hospital discharges and confirm appointments.
• Monitor attendance at therapy, psychiatry, and medical appointments.
• Review PHQ-9 and other screenings to track progress.
• Create, follow up, and close referrals with providers.
• Identify and respond to client risk and coordinate crisis planning.
• Support treatment plan updates based on client needs.
