Job summary

Qualifications

• Bachelor's degree in behavioral health or related field preferred.
• Alternative: HS diploma with 4 years of relevant experience.
• Experience in care coordination or case management preferred.

Responsibilities

• Review clinical records to assess service utilization and treatment plan.
• Document findings in the Care Manager System.
• Coordinate physical, behavioral, and social health services across programs.
• Facilitate access to housing, benefits, employment supports, and substance use care.
• Ensure referrals are tracked and closed with documentation.
• Monitor attendance at therapy, psychiatry, and medical appointments.
• Identify gaps in care and initiate outreach or peer referrals.
• Notify care team of transitions and ensure continuity of care.
• Maintain compliance with agency and Medicaid standards.
• Document reviews daily to ensure timely follow-up and coordination of care.
