Qualifications

• Current State License as a Licensed Practical Nurse, LPN
• Must have a valid driver’s license and automobile insurance liability
• Graduate of a state approved accredited nursing school
• Certified in Basic Cardiopulmonary Resuscitation
• Minimum of five (5) years of nursing experience. Hospice or SOURCE care preferred.

Responsabilités

• Screens referrals and coordinates admission process.
• Coordinates services with providers and handles billing reconciliation.
• Attends team meetings and presents new admits, annual assessments, hospitalizations, and discharges.
• Performs quarterly home visits and PCP visits with required documentation.
• Tracks hospital/ER visits and manages variance plans and follow-ups.
• Coordinates with outpatient therapy centers and home health agencies; reevaluates service needs.
• Uses own vehicle for travel and handles mileage/time sheets for reimbursement.
• Participates in trainings and on‑call rotations; communicates with regulatory agencies.
