Qualifications

• Minimum of five years of nursing experience.
• Graduate of a state approved accredited nursing school.
• Current State License as a Licensed Practical Nurse (LPN).
• Certified in Basic Cardiopulmonary Resuscitation (CPR).
• Must have a valid driver’s license and automobile liability insurance.

Responsibilities

• Screens referrals and coordinates admission processes.
• Coordinates services with providers and handles billing reconciliation.
• Conducts in-home assessments and quarterly visits.
• Tracks hospital/ER visits and maintains records.
• Travels using own vehicle and arranges transportation as needed.
• Trains new hires and participates in on‑call rotation as required.
