Kidney Care Center Management LLC in Joliet, IL is seeking a Care Manager - Licensed Practical Nurse for an in-person role. The care manager will support RPM, CCM and PCM programs, coordinating between corporate and clinic teams to enroll eligible patients and maintain program integrity.

The role requires an LPN license, strong communication skills, HIPAA compliance, EMR proficiency, and the ability to manage chronic disease patients through regular contact and care plan oversight.

Competenze

• Certification as a Licensed Practical Nurse (LPN) is required.

Mansioni

• Assist patients with managing blood pressure and chronic conditions through daily monitoring and calls.
• Follow up monthly with an assigned patient group via phone.
• Review vitals and medical history to support the care plan established by the provider.
• Record patient demographics, medical issues, medications, and risk factors to create a clinical summary in the EMR.
• Communicate with practices/providers when readings exceed parameters.
• Register chronic care management and PCM in the EMR and ensure compliance with Medicare programs.
