Kidney Care Center Management LLC in Joliet, IL is seeking a Care Manager - Certified Medical Assistant to support RPM, CCM, and PCM programs. You will liaise between corporate and clinic teams to ensure eligible patients are enrolled in all applicable programs.

The role requires assisting patients with chronic conditions, documenting in EMR, and coordinating with healthcare providers. Full-time and part-time positions available, with on-site responsibilities.

Qualifications

• Certification as Medical Assistant (CMA or Registered MA) is required.
• High school diploma or equivalent is required.
• Minimum 1–3 years’ work experience in healthcare with chronic disease is preferred.

Responsabilités

• Assist patients with managing chronic conditions through daily monitoring and calls.
• Follow up monthly with assigned patient group via phone.
• Review vitals and medical history to support care plans.
• Record patient demographics, medical issues, medications, and risk factors in EMR.
• Communicate with Practices/Providers when readings exceed parameters.
• Register Chronic Care Management and Principal Care Management programs.
• Audit care plans for accuracy and compliance.
• Participate in education seminars and program meetings.
