Job summary

Qualifications

• Active Illinois license as RN or LPN.
• Minimum 1 year nursing experience.
• Care management experience preferred.
• Ability to educate patients on health topics.

Responsibilities

• Educate patients on lifestyle changes and disease management.
• Collaborate in selecting and maintaining patient education materials.
• Assist in care transitions from hospital to primary care.
• Use clinical orders for preventive and chronic disease care.
• Review meds, reconcile, educate and coordinate.
• Monitor site operations for safety and efficiency.
