Job summary

Allegheny Health Network is seeking a Medical Assistant / LPN to support population health management and care coordination in Pennsylvania. The role includes assisting during patient visits, conducting pre-visit planning, and helping patients manage chronic conditions using motivational interviewing.

You will collaborate with care teams to close gaps, document encounters, and ensure efficient clinic operations while upholding HIPAA and privacy standards.

Qualifications

• High School / GED or completion of a Medical Assistant program or LPN license.
• Current PA LPN licensure or multi-state licensure through the enhanced Nurse Licensure Compact (eNLC) or Medical Assistant Certification.
• 2 years of Medical Assistant experience.

Responsibilities

• Actively participates in population health outreach through identification of patients with gaps in care; contacts patients and facilitates closing gaps in care.
• Ensures pre-visit planning by performing chart and data reviews in advance of patient visits.
• Demonstrates expanded rooming protocol by assisting the provider and activating standing orders.
• Accurately documents elements of the clinical visit under supervision and assists with order entry.
• Uses motivational interviewing to coach patients toward self-management and activation.
