Job summary

Campbell Street is seeking a compassionate Licensed Practical Nurse to join our Midwest facility. You will conduct focused assessments, participate in drafting the resident-centered Plan of Care, and communicate needs clearly to the interdisciplinary team.

You will provide nursing care, monitor residents, document thoroughly, and ensure medications are administered safely while upholding HIPAA and safety standards.

Qualifications

• Completion of a professional nursing education program approved by the state board.
• Current Practical Nurse license for the state where the facility is located.
• Current CPR and emergency care certification.
• Preferred: experience in skilled nursing or assisted living.

Responsibilities

• Conduct focused assessments and draft the Plan of Care.
• Communicate resident needs to the care team and report changes.
• Provide nursing care, ensure safety and HIPAA compliance.
• Ensure proper documentation, medication administration, and incident reporting.
• Promote resident-centered care and quality of living.
